Whilst making up 7-5/8" casing joint ready to wash last joint to bottom. The 2" fill up hose & swing which were made up to the circulating swedge in the top of the casing, backed out at the connection to the swedge & the fell approximately 45 feet to the rig floor. Although personnel were working on the rig floor there were no injuries."Please forward this information onto drilling rigs and Workover rigs and advise this incident is to be reviewed by all crews, and request that evidence of the review is captured in their daily summary or weekly safety minutes.

Why wasn't the hose secured to the bales with a safety cable?

Wow that's scary something similar almost happened to my hubby. The idiot operating the crane kept hitting override and whatever was being hoisted fell he barely stepped back in time and there was a crater in the ground where he was standing and they didn't even file a near miss I was heated!
