Image Reported By Moscow TimesA weekend oil spill at the Trebs and Titov field operated by a joint venture between Bashneft and LUKoil saw about 2,200 tons leak across 5 square kilometers of Arctic terriotry, Interfax reported Monday.

The 25-meter high gusher that began on Friday was not brought under control for more than a day. Investigators looking into the incident have not yet ascertained its cause but a regional official said that human error was to blame.

"The accident happened as a result of insufficiently prepared teams, a lack of essential equipment — in other words it was local disorganization," said Vladimir Tsybin, head of the local administration's department of natural resources and ecology.

What a tragedy.

The one thing that draws my attention is the fact that "insufficiently prepared teams"  are always taking the rep. I agree that some field personnel aren't as well-trained as it needed, but in the other hand, where are the  guys responsible to get their employees trained?

Once I heard a man saying that we are responsible for what we do and for what we don't do. Putting it in simple words, if their superior knew that his staff wasn't capable to do the job and he didn't do anything to stop it from being done, then he was responsible for whatever happened too.

Should they stop looking for some one to put the blame on and start trainning his workers better?!
