Crane Accident

Last week at the Chevron Refinery in Pascagoula a incident occurred when a 240 ton crane turned over backwards with no load.

Apparently the outriggers had been retracted on one side of the crane to get them out of a roadway during the night. When the day operator came in the following morning he did not do a walk around, got in crane and swung around in a high boom configuration and turned over backwards. This is a good reminder of the importance of the need to do a walk-around before starting the operation of any crane. Take a couple of minutes to check what might be different before you start work. If we become complacent it can happen to the best of us.

This certainly amplifies how easily human error significantly contributes to an incident and the importance of good, comprehensive and timely communication between shifts/personnel

Someone should have posted a note on the crane levers explaining the outriggers were up....

I think a hair follicle test is in order !! OOPs wont fly on this one and sh** can only roll in one direction on this one.. Men be careful out there wheres your spotter on a 240 ton aint no (stinger) !!!

It is a bad thing this happend,but it is a wake up call for all us crane operators.
Even for the ones like me that work on a crane that is on a jack up barge.
Here we see that a toolbox meeting or a good handover is in place

No excuse for this to of happened. The "operator" that was going to be using the crane FAILED to do a basic pre-use inspection.

Nothing different than a aircraft pilot doing a pre-flight inspection or a trucker doing a pre-trip inspection...a simple walk around look/see without a coffee cup or cell phone is priceless.

Taking a team approach to prevent something similar...beyond a sticky note...a simple lockout device on the cranes master switch or door, with a note attached.

Sorry, this is not interrupted .... just like its not 240 tons .. but 24 tons.
Do not be angry ..... I am a materialman onshore. So I see him trim, this 24 tons. If 200 tons of no wheels. But the chain wheel.

From what I see, this never happened at my place of work onshore in because jack hidroloic down one / get into the floor. If operators do not panic, he will play his tower crane so it will not happen like this.

I have ran a LOT of truck cranes I cant beleve a operator would just get in the cab and start the machine and swing it around he must have not even done a pre run cheak things like engine oil and hydrolic fluids cables ect you would have to be really be off your game not to see some thing as the legs not out and down tighly and level on there pads !!! along with warning tape and cones in the swing area And I cant get a job back in this area !!!
