Adrian, with top drive rigs, I have noticed that the age old practice of rotating breaks on trips has gone out the window on most of the rigs I'm around in the TX Panhandle. This is not good practice. I once had to literaly hold a hands brains in his head until paramedics arrived when a tong latch broke due to over torqued drillpipe. Another thing is that if the proper make-up torque is not applied @ surface, then the pipe will nearly always over-torque downhole. By using proper make-up torque surface, properly cleaning and doping the connections and rotating breaks on trips, these problems should be alleviated.
