Can someone please explain what is going on with all these refinery problems?

These last 3 months have been awful in the oil and gas industry. This last week I heard about leaks, to refinery explosions in Texas to my 'ole man' just calling me 10 minutes ago letting me know about another explosion in Texas. What is going on? Why is there all these problems all a sudden? At first I thought it is all a conspiracy but now I am thinking something is not right at all. Any feedback appreciated
