Chain Breaks Nearly Killing Offshore Workers

When this huge chain snaps under tension it makes you wonder why these workers were standing behind it?

It is obvious this team DID NOT do a 'task based risk analysis' of the operation before the job.

Did the chain break?

Correct me if I am wrong.

Watching the video, it appears the chain at the 'top' of the picture came loose from whatever had been holding / restraining it.  Maybe a 'sling' or some other device to maintain 'tension' during spooling.

Whatever, these people should not have been in this position.

A pre-job risk analysis (or whatever terminology you use in your area) should have identified all the areas of 'risk' and 'hazard' with this job.

From the limited 'video' I can see other 'violations' of 'common sense', let alone SAFETY.

That's correct Alan-I actually do not know if the chain broke or came loose. This apparently  was a Norwegian vessel I am told. I agree and I saw similar to this here in the US nearly everyday while drilling. Examples would be lifts in the V door from the Catwalk-constantly telling my hands to get out from under the load (or at least stand to the side as long as possible)

Another instance is rig move and lowering the derrick-every rig move I have seen, hands stand directly under the derrick the entire time holding cables like it's doing something and me ALWAYS telling them to "Get out from under there"-if a cable looks like its hung up-THEN we will move in and fix it!

At some point self-preservation has to kick in for these guys' like they cannot fathom that things break and fall!

I always hope sharing this stuff will make a difference somewhere to someone.

Yes Curtis it does help, I have used your site in safety meetings. Back when I was drilling and Pushing

anyways. You do help, we just never tell you so with that said....Thank you
