Chain snapped on draw works, brakes failed, 162,000lbs hit the floor.

The main drive chain for the draw works broke.
( wasn't getting oiled properly - oiler was clogged with rug, silicone, and garbage.) The blocks started falling, Driller hit the brakes and they failed. Brakes were wet from a leaky coolant line.

Toolpusher was informed the night before while cutting the drilling line that something was wrong with the drawworks chains.

I saw that happpen once. I was relief driller filling out my books when the shift driller asked me if I wanted to start pulling out of the hole....like I was going to relieve him early...I told him set back the kelly and I'd be out there. He picked up the kelly high and let her sickle down and hit the brakes..the brake came apart and everything hit ground..missed the floor hands but made a mess. By the time we got everything back up and running, the pipe stuck and we ended up losing the string....just a whole lot of wrong on that job. Heh heheh

I had this kind of thing happen, was brake plash system crew had run in 5600m's 3.5"DP , just picked up the kelly and started drilling cement driller could not hold the string and the blocks where layed out on the catwalk not as much damage though.
